H1N1 UPDATE: Volunteer Vaccination Date Set

The Cook County Department of Public Health (CCDPH) has informed District 57 that the H1N1 Volunteer Vaccination date for all District 57 schools will be Thursday, December 10, 2009.

Information packets with Consent Forms were sent home with all students on Monday, November 30. Information packets are also available for download on the District web site (www.d57.org).

Completed Consent Forms should be returned to your child's school no later than Friday, December 4. A completed Consent Form is required before any student will receive a vaccination.

Vaccination hours will be approximately 9am to 3pm on December 10th. If parents would like to be in attendance for their child's vaccination, TENTATIVE schedules will be provided by the schools. CCDPH has informed us that any schedule is considered TENTATIVE and could be delayed by more than one hour due to vaccine shipment arrival which occurs that morning.
