Enrollment Intake
Welcome to District 57's Registration Intake.
This short, online form will allow parents to communicate information to staff prior to visiting their enrolling school in-person to prove residency, and will be checked for accuracy during the visit.
Please note that completion of this online form does not guarantee admission. Guardians will need to prove residency during Enrollment Intake, and information regarding necessary proof can be accessed here.
Finally, please note that parents will also have to take part in the District's larger Online Registration to complete the process. Parents will not have to enter data twice. During the separate Online Registration parents will be asked for appropriate payment and additional information. Online Registration for the following school year is normally held in May.
Intake Steps:
Step 1:
Submit information through this online form. This form sends data to District employees for processing.
Step 2:
Print out and complete these three forms. The Home Language Survey and Race & Ethnicity forms are required by the State of Illinois.

Step 3:
A parent or guardian should bring the forms from Step 2, as well as proof of residency (see below) to your child's enrolling school. Please note: students are not required to attend.
Proof of Residency
(Required for all new students and must be completed in person at the enrolling school):
Students who attend school in Mount Prospect School District 57 must reside within its boundaries on a permanent basis. If you are enrolling your child with us for the first time, or have changed your residence since registering, you will need to verify that your child still resides within the district boundaries and you must provide new proof of residence and a signed Proof of Residency Form to the school office administration as indicated below.
At the time of enrollment, you must present required documents from EACH of the steps below.
You must also bring in the original birth certificate for the student you are enrolling.
Part 1

- Driver's License (original required)
- State Identification Card (original required)
Part 2
(provide one)
- Lease document
- Mortgage document
- Tax bill and proof of payment
- Letter of Residence Form 7:60AP2 (need one of following three):
- Gas bill (Current and previous month)
- Electric bill (Current and previous month)
- Water bill (Current and previous month)
- Telephone bill (Current and previous month)
- Cable bill (current and previous month)
- Credit card bill (current and previous month)
- Other bill (current and previous month)
Post Office boxes do not meet residency requirements. Only street addresses are accepted.
If, at any time, a student’s residence is in question, MPSD 57 will investigate. The District may ask for additional documents for verification. If a student is not living within the MPSD 57 boundaries full time, or if a student’s living arrangements do not agree with statements provided by a parent or legal guardian, the student will be excluded from attending schools in the Mount Prospect School District.
