Registration
RETURNING STUDENTS

Online registration for returning students is required to be completed annually. In order to access online registration, an access code (Snapcode) is required which is emailed to parents/guardians on or before May 1st. To access a link to online registration click here. For technical assistance with online registration, contact the Infosnap Support Line at (866) 986-1601 ext. 7095 or send an email to onlineregistration@d57.org.

Registration Steps

  1. InfoSnap Snapcode emailed to parent/guardian on or before May 1st. If you do not receive this email, check your SPAM folder OR email your contact information to onlineregistration@d57.org.
  2. Parent/guardian completes online registration and pays registration fees online.
  3. Note: Discounted online registration begins May 1 and runs through June 12.
NEW STUDENTS

According to the Illinois School Code, all students attending Mount Prospect School District 57 must be legal residents of the school district. In the event of separation or divorce, court documentation declaring the student’s residence to be the address of the guardian or custodial parent must be presented. Before enrolling your student, you will need to prove residency by completing the Residency Verification form and submitting the following required documentation to the school office. Only one Residency Verification form per family is required to be submitted to the oldest student’s school.

STUDENT INFORMATION (PROVIDE ONE PER STUDENT)
• Birth Certificate/Passport

STEP 1 (PROVIDE ONE)
• Driver's License
• State Identification Card

STEP 2 (PROVIDE ONE)
• Lease document
• Mortgage document
• Tax bill and proof of payment
• Letter of Residence Form 7:60AP2 (need one of following three):
  1. Letter of Residence from Landlord in Lieu of Lease (E1)
  2. Letter of Residence When Person Seeking to Enroll Student is Living with District Resident (E2)
  3. Letter of Evidence of Non-Parent's Custody, Control and Responsibility of Student (E3)

STEP 3 (PROVIDE TWO CONSECUTIVE MONTHS OF TWO BILLS)
• Gas bill (Current and previous month)
• Electric bill (Current and previous month)
• Water bill (Current and previous month)
• Telephone bill (Current and previous month)
• Cable bill (current and previous month)
• Credit card bill (current and previous month)
• Other bill (current and previous month)

Enrollment and Registration Steps

  1. Parent/guardian must visit school to prove residency by completing Residency Verification form and submitting required documentation as detailed above.
  2. Upon approval, Snapcode issued for access to online registration.
  3. Parent/guardian completes online registration and pays registration fees online.

If, at any time, a student’s residence is in question, the District will investigate. The District may ask for additional documents for verification. If a student is not living within the District boundaries full time or if a student’s living arrangements do not agree with statements provided by a parent/guardian, the student will be excluded from attending schools in Mount Prospect School District 57.


STUDENT FEE WAIVER

Mount Prospect School District 57 uses the qualifying USDA Income Eligibility Guidelines to determine if a student qualifies for a fee waiver. To qualify for a fee waiver, a student fee waiver application must be completed, submitted with all supporting documentation to the school office, and approved. Student Fee Waiver must be approved prior to receiving services and/or supplies. The Kids’ Corner program is not eligible for a student fee waiver.
